Providing care as a Private Professional Fiduciary is our passion. Fiduciaries practicing in the state of California are required to be licensed by the Professional Fiduciaries Bureau under the California Department of Consumer Affairs and must continually evolve with additional education requirements. Many fiduciaries have had previous careers as attorneys, CPA's, bankers, social workers, or healthcare providers. The responsibility is unique to each client and their needs, which often evolve or have phases. Passage Planning views it as our responsibility to listen, guide and serve each client uniquely.

A fiduciary's role may include roles as manager, trustee or guardian. It is also a nurturing bond of trust, concern, and attentive care-giving. A fiduciary seeks to support mental and emotional well-being; reduce the stress of changing circumstances or unexpected events; and, most importantly, help each client, and their families, enjoy a fulfilling life.

The Role of Passage Planning as Your Partner in Strategic Planning for Seniors
Strategic Planning/Logistical Concierge Services for Seniors 

Aging in Place Services for Seniors
(client resides in private home, condominium, apartment, assisted living accommodations) 

A client centered approach to development of a daily logistics strategy. This should include the client and existing caregivers/family members including: 

Connecting Seniors to Their Community 
-Social engagement in community classes, programs, development of friendships, places of worship
-Accompany client to appointments (medical, social, vendors)
-Transportation strategies
-Development of interests/hobbies/group interactions

Finances and Technology Access
-Technology use/minimization/simplification and security against digital fraud (phone, tablet, computer and online applications)
-Financial access and potential stumbling blocks

Home and Wellness
-Medication review and pharmacy troubleshooting; follow up with client access to medical team
Interface with other professionals including geriatrician, internist, case managers, nurses, medical social workers, real estate professionals, housing staffing, therapists, home care aids and caregivers
-Reviewing general client and home care (Hygiene, basic tasks, yard care and safety)
-Observing client in home for possible referral to therapeutic aids (physical, occupational, emotional and spiritual support)
-Facilitating Family and friend connection, travel arrangements
-Support of specific nutrition/physical exercise plans

Task Prioritization and Completion
-Engaging services for task completion including seeking estimates and payments
-General home safety checks
-Assessment of obvious home repairs and developing a task list via budget
-Project management for home repairs, installations, landscaping renovations (walkways, accommodations on the property, storage)

Task Management 
(Client may live independently or in an alternate home/facility)

Helping seniors and their caregivers with daily life tasks accompanying the senior to make their own choices while following the recommendations of professionals on their team. 

Tasks often include: 
-Visits to financial institutions, professional offices, creation of asset inventories with protected access
-Assisting with nutritious grocery shopping choices
-Personal shopping for necessities
-Securing materials for projects
-Home declutter for safety/peace of mind/additional space for attendants and visitors
-Estate clean outs/preparations for estate sale or auction 
-Creation of space for private therapeutic use (occupational, physical, massage, etc)
-Facilitating home trash removal/recycling/donation/storage/hauling/hazardous waste
-Supervising installation of medical devices, in-home safety equipment, lighting, landscaping, walk-way improvements
